Once you schedule your first appointment with Healthy Smiles Dental you will receive a link via email and/or text to complete your new patient paperwork. If you prefer to fill the forms out on paper we have a printable PDF version available or you can arrive 20 minutes before your scheduled appointment to fill it out in office. Please remember to bring a list of current medications and a copy of your insurance card.
